If we are to provide your mediation, we will ask you to fill out two brief forms prior to your session. You are welcome to complete these now. There is no obligation or fee until we have spoken and agreed to provide your mediation services.

  1. Agreement to Mediate We ask you to read and sign the Agreement to Mediate. This indicates that you understand and agree to the groundrules of mediation.  It does not commit you to anything else.
  2. Intake Form: Please select Family (Divorce, Separation, Parenting) OR Other Type of Mediation This provides us with contact information for yourself and the other party(ies) involved.
